Club Soleil Tanning Freeze program allows you to maintain your current pricing and an active account status for just $5.00 per month (plus tax). I understand that my account will be debited $5.00 per month (plus tax). My notice to freeze my membership must be completed 14 days before my draft date. At the end of the freeze period chosen, my full monthly draft amount will continue to draft as stated on my original membership agreement. By filling out this form, you are not cancelling your Club Soleil Tanning membership.
